Need to generate order form with thumnails and file names

Does anyone know how to, or a software that can do this for me? I want to create an order form that shows thumnails with the filenames. I've already created a folder with thumnails for this, but don't want to have to type them in manually.

    This should be a cinch -- if you use Photoshop that is.

    Try this - Click File > Automation > Contact Sheet II

    Then choose the folder that the thumbnails are in and set your options the way you want. It will generate an 8.5"x11" contact sheet with file names under the thumbnails.

    Thanks for the help, but I was thinking more of a form type - with the thumnail in the left column and then all the order options to the right in an excel type form. Basically so it looks like a fill-in order form.

    If you have adobe InDesign you can use Data Merge. Tutorial here:

    I believe that Microsoft Publisher has a similar feature, but it has been a LONG time since I have used it. Sorry I couldn't help.

    I Really appreciate the attempt. I was able to use the contact sheet II as a starting point. I can use it to print succession shots for our pitchers. They really like to see them all on one page lined up.
