The first step to create a contact page is to go to your Profile > About Me and add a contact email address. Once you've added a contact email address, an email will be sent to that address asking you to verify you own it. Click the link in the email to verify.
Your contact email address won't work until verification is complete. You may also want to pick a profile photo while you're on your Profile page.
If you provided an email address and responded to the email sent to verify it and your contact form still doesn't work to send an email to you then I believe you will need to contact the support heros to take a look - http://help.smugmug.com/customer/portal/emails/new.
I've just sent a test to your contact email address, using the contact form with the subject: Testing Contact Form
You are currently using an AOL email address. AOL often thinks SmugMug emails are spam. And, the way forms work they send the message from the visitor email VIA SmugMug. So, you will want to check your SPAM/junk folder and make sure any messages from @smugmug.com are allowed to make it through to you. Your contact email address is verified.
