I've had great service from www.customsportscards.com. I deal with Dave Farpelha. Get the matte card stock, it's nice.
Sadly SmugMug doesn't do trading cards. (yet?)

Bay Photo now does cards, memory mates, magazine covers. I do printing direct through them and cut out Smugmug. Bay doesn't offer all their products through smugmug. The cards are good stock and decent selection of templates. They have great customer service and very cheap shipping.
