i know some use a neckstrap or shoulder strap. my favorite way to carry a camera is with a handstrap.
on my 20d, i used the hakuba handstrap, it attaches to the bottom of the 20d in the tripod socket
here it is on my 20d

Doesn't that make portrait orientation difficult?

for this photography, not hardly, no different than without a handstrap.
i've never had difficulty shooting portrait orientation without a grip, btw. and with the canon batteries lasting for like, 1000 shots or 9 days (:lol3) it's hardly necessary to schlep the grip.

Yeah, I didn't mean the grip. I mean yer hand's strapped in for landscape orientation. You slip your hand out of the strap and over it for portrait? It doesn't look like the 20D has the separate shutter release button for portraits.

I'll second the recommendation for the hand strap. Haven't picked up anything
for the 1dmkII yet.
Thanks for the recommendation. I thought I might be stuck with the Canon
version :D

I have the Canon handstrap on the 1DII. It is very hrd t set just the way I like it. The adjustments take a little time but stay in place once you find the sweet spot.

I like the idea, since the grip seems very natural to me, but what would it be like long term with say, the Canon 70 - 200, as opposed to my normal Tamron 28 -75 walk around? I can see possible hand or muscle cramps in my future. Of course, for only $18, what do I have to lose?

heck, you'll see, 'gus.. it's really hard to deplete the battery on the 20d. oh but buy a spare one, anyhow, to have. and aftermarket batteries are fine....

I need another neck strap in case of breakage, I suppose they are easy to find. And I will procrastinate until my camera falls and breaks.
Is there a cheaper backup? (around the neck, the only time my hand is not on my camera is when I really have my hands full, otherwise I try to use both for insurance.)
I don't have any free hands, even with a backpack. Andy, do they also sell hands, I have always wanted an extra.
