Why won't this show up?

why doesn't my custom header show up on my "side" galleries such as:

my scanning service page: http://www.smugmug.com/gallery/1614540
and my prices page: http://www.smugmug.com/gallery/1131843
AND my contact page: http://www.rossfrazier.com/gallery/1434761

any ideas?!?! I'm so confused....

you have "hide owner" turned Yes, or appearance turned to "smugmug" or both.

(gallery tools>customize gallery>)
