We now have the contact form on the password page that can be used to email the site owner. Can this form please be documented so that we can use it on our own "about me" pages?

Not sure I understand the request. I recently setup a contact form for website visitors for me and my partner. I used Google Apps Forms and spreadsheets. Then I setup a Gadget in iGoogle so I know when I get a form submission. Easy to do and free.
Are you talking about that kind of contact form?

Hey Phil,
Yes, that's kind of what I'm asking for. If you attempt to go to a password protected gallery and then click on "contact this photographer", then you will see the form pop-up.
Your solution looks pretty good too... I may give that a try. Thanks.
